By submitting the form, you agree that your personal data may be collected, used and disclosed by ONE15 Marina Club and its related organisations, service providers and agents for assessing the suitability of your membership application with the Clun, the management and administration relating to your membership if you successfully become our Member and for the purposes set out in the Club’s Privacy Policy which is found on the Club’s website: https://one15marina.com/privacy-policy/, and may include the taking of your photographs and videos during the Club’s events and activities for the Club’s publication.

• You warrant that where you have disclosed personal data of third parties to the Club and its related organisations, service providers and agents in connection with the abovementioned purposes, you have obtained the prior consent of such third parties for the Club and its related organisations, service providers and agents to collect, use and disclose such personal data for such purposes, in accordance with any applicable laws, regulations and/or guidelines.

• You consent that, the Club may contact you via mail, electronic mail, telephone (call or WhatsApp), to inform you about our announcements, services, events, and membership benefits offered by the Club or third party service providers, partners or merchants that we think may be of interest to you.

• Please refer to the Club’s Privacy Policy pertaining to how your personal data is collected, used and disclosed for the aforesaid purposes and how you can get connected with our Data Protection Officer.

Applicant's Declaration *

Relying on the information received and such other information as ONE15 Marina Club (the “Club”) may request from the principal Member, and subject to the payment of the subscription fee (based on family rate), a supplementary card may at the discretion of the Club be issued to the above named supplementary card applicant, and which upon presentation to the Club shall entitle the supplementary cardholder to enjoy and use the facilities of the Club.

• Any breach of the Club Rules and Regulations and Bye-laws by the supplementary cardholder shall be deemed a breach by the principal Member, and the Club shall be able to take whatever remedial action or action necessary to protect its rights and interests.

• Principal Member shall be liable to the Club for payment of all dues and charges incurred by the supplementary cardholder and his/ her guests for the consumption of goods, food and services in the Club and/or any charges or liabilities arising as a result of a breach of the Club Rules and Regulations and Bye-Laws by the supplementary cardholder and his/ her guests.

• Principal Member shall indemnify and keep the Club indemnified against any demand or claim by and any loss or damage suffered by the Club arising out of the issuance of the supplementary card to the applicant or any breach by the supplementary cardholder of the Club Rules and Regulations and Bye-laws or in relation to any misrepresentation or incorrect information provided by the principal Member to the Club in respect of this application for a supplementary card.

• Principal Member shall indemnify the Club against all claims, proceedings or actions made by the supplementary cardholder and his/ her guests against the Club in respect of any matter whatsoever.

I acknowledge that my use of the facilities at ONE°15 Marina Sentosa Cove Singapore (“the Club”) is a privilege and not a right. In alignment with the Club’s Rules and Regulations and Bye-laws accepted by the Principal Member upon application for membership, I hereby accept and agree to the following personal responsibilities while on the Club’s premises:

• I will abide by all Club Rules and Regulations and Bye-laws.
• I will conduct myself courteously and respectfully towards all Club Members, guests, and staff.
• I will treat the Club’s property and premises with care and respect and acknowledge that I may be held responsible for any loss or damage caused by my actions.
• I will behave in a proper, safe, and responsible manner and adhere to appropriate dress standards in all areas of the Club.
• I will use the lobby, entrances, stairways, and all common areas solely for their intended purposes and will not loiter, congregate, or use these areas as play spaces.
• I will leave “nuisance items” (as determined by the management) at home and not bring them into the Club. If caught using such items, they may be immediately confiscated by the management.
• I will not bring food or beverages from external vendors into the Club.
• I will refrain from inappropriate “public displays of affection”.
• I will abstain from any illegal activities on the Club’s premises, including but not limited to smoking, consumption of alcohol (where prohibited), possession or use of illegal drugs, weapons, or any unlawful substances or items.
• For the avoidance of doubt, I understand that I am not permitted to introduce or host guests at the Club at any time.

I will exercise common sense and sound judgment at all times. Should there be any uncertainty regarding acceptable conduct, I will seek guidance from a Club staff member or a responsible adult. I understand that the above responsibilities serve as general guidelines and do not constitute an exhaustive list of acceptable or unacceptable conduct.

I confirm that I fully understand and agree to comply with the responsibilities set out above. I further acknowledge that my privileges are directly linked to the Principal Member’s membership. Depending on the frequency and/or severity of any breach, one or more of the following actions may be taken at the discretion of the Club:

• A reprimand for unacceptable behaviour
• A verbal or written warning, with notification to the Principal Member
• Removal from the Club’s premises for the remainder of the day
• Temporary or permanent revocation of my supplementary card
• Temporary or permanent suspension or revocation of some or all privileges
• Referral of the Principal Member to Club Management for further action
